Job summary

Maxim Healthcare Services, Inc. in Richmond, VA, is hiring a Licensed Practical Nurse (LPN) to provide in-home care for pediatric and/or adult patients. Starting pay ranges from $30–$35 per hour, with weekly paychecks and a full benefits package.

The role emphasizes nursing process implementation, care planning, medication administration, and coordination with prescribers and supervisors. Requirements include current LPN license, health certificate, PPD/Chest X-Ray, and BLS.

Qualifications

• LPN license in the state of practice.
• Current health certificate as required by facility or state regulations.
• Current PPD or Chest X-Ray.
• Current BLS card.
• One year of LPN experience preferred.

Responsibilities

• Utilizes the nursing process to plan, implement, and evaluate patient care for clients at home.
• Initiates the care plan and requests revisions from prescriber as patient needs change.
• Incorporates therapeutic, preventive, and rehabilitative nursing actions based on diagnosis and goals.
• Intervenes and reports changes in patient status to physician or supervisor.
• Administers medications and treatments as prescribed and documents outcomes.
• Reviews care plans and protocols to identify education, coordination, and discharge needs.

Responsibilities:

• Utilizes the nursing process to plan, implement, and evaluate patient/client care and/or services provided to client(s) receiving contracted services, as required
• Initiates the plan of care when directed and requests from prescriber any necessary revisions as patient status and needs change
• Incorporates therapeutic, preventive, and rehabilitative nursing actions based on patient diagnosis and established goals
• Intervenes as applicable in response to changes in patient/client condition and reports such changes to appropriate physician or supervisor
• Administers medications and treatments as prescribed, as well as, monitors and documents effectiveness of intervention
• Reviews plan of care and program protocols and identifies areas for healthcare teaching/education, care coordination, goal setting and discharge planning needs, as applicable

Requirements:

• Current Licensed Practical Nurse(LPN) License for the state in which the nurse practices.
• Current Health Certificate (per facility Licensed Practical Nurse(LPN) contract or state regulation).
• Current PPD or Chest X-Ray.
• Current BLS card.
• One year prior Licensed Practical Nurse (LPN) experience preferred.
