Maxim Healthcare in Savannah, GA is hiring a Licensed Practical Nurse to provide home-based care for pediatric and adult patients with G-tube, trach and ventilator needs. You will work within Maxim's patient-centered care model to ensure safety and quality outcomes.

The role requires active GA or Compact LPN licensure, BLS, health certificate, and relevant experience; a supportive benefits package and weekly pay are offered.

Qualifications

• Current Georgia or Compact LPN license.
• Current health certificate per facility regulation.
• Current PPD or Chest X-ray.
• Current BLS certification.
• One year prior LPN experience preferred.

Responsabilités

• Utilizes the nursing process to plan, implement, and evaluate patient care for home-based clients.
• Initiates the plan of care and updates as patient status changes.
• Performs therapeutic, preventive, and rehabilitative nursing actions per goals.
• Responds to changes in patient condition and notifies physician or supervisor.
• Administers medications and treatments as prescribed and documents outcomes.
• Reviews plan of care and coordinates education, goal setting, and discharge needs.

Position Details

• Location - Beaufort, Jasper, and Hampton Counties in SC and Chatham, Effingham, Bulloch, Bryan, Liberty, Long, Wayne, Mcintosh, Glynn, and Camden Counties in GA
• Shifts - Day, Night, and Evening

Responsibilities

• Utilizes the nursing process to plan, implement, and evaluate patient/client care and/or services provided to client(s) receiving contracted services, as required
• Initiates the plan of care when directed and requests from prescriber any necessary revisions as patient status and needs change
• Incorporates therapeutic, preventive, and rehabilitative nursing actions based on patient diagnosis and established goals
• Intervenes as applicable in response to changes in patient/client condition and reports such changes to appropriate physician or supervisor
• Administers medications and treatments as prescribed, as well as, monitors and documents effectiveness of intervention
• Reviews plan of care and program protocols and identifies areas for healthcare teaching/education, care coordination, goal setting and discharge planning needs, as applicable

Requirements

• Current Georgia or Compact Licensed Practical Nurse (LPN) License
• Current Health Certificate (per facility LPN contract or state regulation).
• Current PPD or Chest X-Ray.
• Current BLS card.
• One year prior Licensed Practical Nurse (LPN) experience preferred.
