Job Summary: The Home Care Licensed Practical Nurse is responsible for providing direct nursing care as prescribed by the physician to acutely ill and/or chronically ill patients in their homes, recording clinical and progress notes, and coordinating other patient care activities/services to provide the highest quality of patient care in the geographic service area. She/he works under the supervision of the Registered Nurse and/or designated supervisor, within the scope of practice defined by state laws and agency policies.

Specific Job Duties/Responsibilities:

• Provides high-quality clinical services within the scope of practice, following all infection control procedures.
• Documents patient visits thoroughly in the electronic medical record using the agency’s electronic visit verification program.
• Participates in care coordination, including reporting patient needs and changes, attending case conferences, and communicating effectively with patients, families, physicians, and team members.
• Maintains confidentiality of patient health information per agency policy.

Requirements include current LPN licensure in Tennessee, a valid driver’s license, vehicle insurance, access to reliable transportation, and current CPR certification. Previous home care experience is preferred.

This position is full-time, entry-level, in the health care provider industry, serving hospitals and health care sectors.
