Job summary

Crossover Health is seeking a licensed vocational nurse (LVN/LPN) to join our clinic team in Menlo Park. This role supports patient care coordination, check-ins, and collaboration with specialists to ensure a seamless patient journey within our Primary Health model.

The LVN/LPN will assist with immunizations, phlebotomy, and handling benefit details, copays, and related insurance aspects while upholding our value of patient-centered care in a fast-paced environment.

Qualifications

• Licensed Vocational Nurse (LVN/LPN) with current license and at least 3 years of clinical experience.
• Current BLS (Basic Life Support) certification.
• Experience in fast-paced healthcare settings preferred.

Responsibilities

• Delivers patient care under RN or clinic supervisor oversight.
• Coordinates with care team to support a patient-centered medical home.
• Manages patient check-ins and assists with clinic workflows.
• Provides phlebotomy and immunization services.
• Engages with members to schedule appointments and respond to inquiries.
