LiveWell Homecare Agency, LLC is seeking an Inpatient LPN to provide direct patient care across assessments, planning, implementation, and evaluation of care. You will address physical, psychological, social, cultural, and spiritual needs while reinforcing education and discharge planning.

You will collaborate with the health care team to ensure safe, effective, and compassionate care. Work hours are 7 p.m.

Formación

• Graduate from a Specialty Training Program in Nursing.
• Ability to provide compassionate, patient-centered care.
• Strong collaboration, communication, and critical thinking skills.

Responsabilidades

• Deliver treatments, medications, and diagnostic studies per policies.
• Evaluate the effectiveness of nursing interventions based on patient response.
• Participate in unit-based decision-making and evidence-based practice.
• Mentor new staff and students, and contribute to performance appraisals.
• Promote patient safety, satisfaction, and adaptive responses.
• Precept and collaborate with interdisciplinary team members to meet patient needs.

Role Overview:

Provides direct patient care as an Inpatient Licensed Practical Nurse (LPN), contributing to assessments, planning, implementation, and evaluation of care. Addresses patients’ physical, psychological, social, cultural, and spiritual needs, while reinforcing education and discharge planning. Collaborates with healthcare team members to ensure safe, effective, and compassionate care.

Responsibilities include:

• Delivering treatments, medications, and diagnostic studies according to policies
• Evaluating the effectiveness of nursing interventions based on patient response
• Participating in unit-based decision-making and evidence-based practice
• Mentoring new staff and students, and providing input on performance appraisals
• Promoting patient safety, satisfaction, and adaptive responses
• Precepting and collaborating with interdisciplinary team members to meet patient needs

Qualifications & Skills:

• Graduate from a Specialty Training Program in Nursing

• Strong collaboration, communication, and critical thinking skills

Work hours include 7 p.m. to 7:30 a.m., with every other weekend and holiday requirements.
