Responsibilities include medication administration, patient assessment, documenting care, patient and family education, and collaborating with the interdisciplinary team to ensure high-quality care.

Qualifications

• Must possess a nursing degree from an accredited college or university, or be a graduate of an approved LPN or RN program.

Responsibilities

• Medication administration and treatments.
• Assist with admission, transfer and discharging of residents.
• Document resident care services by charting in resident medical record and department records.
• Educate residents and family members about treatment, rehabilitation, or status changes.
• Adhere to infection control policies and facility policies.
• Consult with resident’s physician regarding plan of care and changes.
• Maintain resident confidentiality and privacy.
• Ensure quality of care by following DOH standards and facility standards of care.
• Collaborate with interdisciplinary team to solve resident problems.
