Creekside Center for Rehabilitation and Healing in Nashville, TN seeks an LPN to join our post-acute care team. You will administer medications, assist with admissions and discharges, document care, and educate residents and families as part of an interdisciplinary approach.

We offer top pay up to $33/hour, weekly and daily pay options, insurance, perks, and a supportive work environment with multiple shifts available: day, night, full time, part time, and per diem.

Qualifikationen

• Nursing degree from an accredited college or university or graduation from an approved LPN or RN program.

Aufgaben

• Medication administration and treatments.
• Assist with admission, transfer and discharge of residents.
• Document resident care in medical records and department records.
• Educate residents and families about treatment and rehabilitation.
• Adhere to infection control policies and facility procedures.
• Consult with physician regarding plan of care and changes.
• Maintain confidentiality and privacy of residents.
• Ensure quality of care per DOH and facility standards.
• Collaborate with interdisciplinary team to solve resident problems.
