Creekside Center for Rehabilitation and Healing in Smyrna, TN is seeking an LPN to join its post-acute care team, delivering medication administration, care planning, and resident support. This role offers flexible shifts (day/night), competitive hourly pay up to $33, and benefits including insurance and special programs.

You will collaborate with physicians, therapists and family members to maintain high standards of care.

Qualifications

• Must possess a nursing degree from an accredited college or university, or be a graduate of an approved LPN or RN program.
• Medication administration and treatments proficiency.
• Documentation of resident care in medical records.

Responsabilités

• Medication administration and treatments.
• Assist with admission, transfer and discharging of residents.
• Document resident care services by charting in resident medical records.
• Educate residents and families about treatment and status changes.
• Adhere to infection control policies and facility procedures.
• Consult with physicians regarding plans of care and changes.
• Maintain resident confidentiality and privacy.
• Support quality of care by following DOH and facility standards.
• Collaborate with interdisciplinary team to solve resident issues.
