Job summary

Sarah Bush Lincoln seeks an LPN for physician practices to support providers during patient visits and procedures. The role includes phone triage, data collection, patient education, and coordination of tests and referrals.

Shifts run 7:00am–7:00pm over three 12-hour days with weekend and holiday rotation; compensation starts at $23.87 per hour and ranges to $37.00, based on experience.

Qualifications

• High School diploma required.
• BLS or CPR certification within 90 days of hire.
• Graduate of Accredited LPN Program.
• Licensed Practical Nurse - Illinois Department of Financial and Professional Regulation

Responsibilities

• Answers phone calls from patients, hospitals, extended care facilities, pharmacies and physician offices and directs the information to the appropriate provider as needed.
• Assists providers with patient exams/treatments/procedures.
• Assists the providers by performing patient calls regarding test results and follow-up care as instructed by provider.
• Collects pertinent data and information relative to the patient's health or the situation.
• Coordinates follow up care including scheduling of tests and referrals as ordered by the physician in a timely and accurate manner.
• Documents appropriately and concisely assessment, care provided, and data collection in the medical record.
• Ensures that pre-certification of tests/consults is obtained.
• Performs and documents laboratory duties which may include phlebotomy, specimen collection and performance of routine basic waived testing.
• Submits prescriptions and refills to the pharmacy ordered by the physician in a timely and accurate manner.
• Teaches patients and family members about the plan of care and documents education.

Responsibilities

Estimated Compensation Range $23.87 - $37.00 Pay based on experience
