Job summary

Elara Caring seeks a Licensed Practical Nurse (LPN) to provide compassionate, in-home nursing care across Kansas City and surrounding areas. You will follow care plans, monitor patient status, and coordinate with the care team to prevent hospitalizations while supporting families in managing health needs.

Requirements include a Missouri LPN/LVN license, a nursing program graduate, and strong communication skills. Travel about half of your time is expected; telecare options are not indicated.

Qualifications

• Current LPN/LVN license in Missouri.
• Graduate of an accredited nursing program.
• At least 1 year of experience in acute care/home health/hospice preferred.
• Strong communication and interpersonal skills.
• Reliable transportation with travel ~50%.

Responsibilities

• Deliver in-home nursing care aligned with patient care plans.
• Review and follow Plan of Care prior to each visit.
• Communicate patient updates and changes in condition.
• Assist with medication management and documentation.
• Educate patients and families on self-care techniques.
• Identify safety concerns and help prevent hospitalizations.
• Collaborate with RN Case Managers, physicians, and care team.

As a Licensed Practical Nurse LPN, you'll contribute to our success in the following ways:

What is required?

• Graduate of an accredited nursing program
• At least 1 year of experience in acute care (home care, home health or hospice preferred)

• Ability to manage time effectively and stay organized in the field
• Knowledge of healthcare regulations and patient care standards
• Compassionate, patient-first mindset
• Reliable personal transportation (travel required ~50%)
• Ability to meet health screening and compliance requirements

You will report to the Clinical Manager or Branch Director for Personal Care Services.
