Job summary

Qualifications
- Must be a Licensed Practical Nurse with a current PA license.
- Prior experience in a healthcare setting is preferred.
- Excellent communication and interpersonal skills for collaboration with the care team.
- Compassionate and respectful attitude toward residents, families, and colleagues.
Responsibilities
- Provide high-quality nursing care to residents in our Personal Care Home.
- Develop and implement individualized care plans with the interdisciplinary care team.
- Monitor and report changes in resident condition and address concerns.
- Educate residents, families, and caregivers on care instructions.
- Maintain accurate records of resident care and interactions.
