Job summary

Deer Meadows Rehabilitation and Nursing is seeking a Licensed Practical Nurse (LPN) to provide direct nursing care to residents and supervise day-to-day nursing activities performed by nursing assistants. Responsibilities include observing residents, administering medications, documenting health data, and reporting changes promptly.

Qualifications

Licensed Practical Nurse with current state license.
Active CPR certification.
Excellent technical, assessment and documentation skills.
Ability to lift 50 pounds and stand/walk >4 hours.

Responsibilities

Observe and manage the resident's health according to individual needs.
Provide routine care, ensure comfort, and report changes in health and behavior.
Collect, track, maintain, and review patient medical history and vital signs.
Administer medications and injections, provide wound care, perform standard diagnostic procedures.
Attend to other special projects and duties as assigned.
