Job summary

Fairmount Homes, Inc in Ephrata, PA is seeking a Licensed Practical Nurse (LPN) / Charge Nurse for the Personal Care neighborhood. This part-time role covers 24 hours/week, with day shifts on Mondays and Thursdays plus alternating weekends. A $1,000 sign-on bonus is offered.

The LPN will provide clinical leadership, oversee daily operations, and ensure compassionate care for older adults while guiding Resident Assistants and maintaining regulatory compliance.

Qualifications

What You'll Do

As an LPN / Charge Nurse, you will provide clinical leadership and compassionate care while supporting the daily operations of the Personal Care neighborhood.

Responsibilities include:

• Administering medications and treatments according to physician orders and facility policies
• Monitoring resident health and responding appropriately to changes in condition
• Making resident rounds to ensure safety, comfort, and quality care
• Completing required clinical documentation, progress notes, and resident records
• Supervising and supporting Resident Assistants while fostering accountability and teamwork
• Communicating resident needs and updates to team members and leadership
• Assisting with resident assessments, admissions, transfers, discharges, and end-of-life care
• Receiving and processing physician orders in accordance with facility policies
• Maintaining infection prevention, safety, and regulatory compliance standards
• Completing incident and accident reports as required
• Participating in team meetings and quality improvement efforts
• Supporting residents and families with compassion, professionalism, and respect

Qualifications

• Current Pennsylvania Licensed Practical Nurse (LPN) License required
