Job summary

Alps at Wayne Rehab & Care Center is seeking a Per Diem Licensed Practical Nurse (LPN) to provide hands-on nursing care to residents and support the unit’s clinical and leadership needs.

You will administer medications, perform assessments, and collaborate with interdisciplinary teams to maintain high-quality care. The position requires an NJ LPN license and the ability to work independently within a care team.

Qualifications

• Current NJ LPN license required.
• Graduate of an accredited LPN or vocational nursing program.
• Ability to work independently and collaboratively as part of a care team.
• Must pass TB screening and physical.
• Strong communication, organizational skills, and attention to detail.

Responsibilities

• Provide direct nursing care according to individualized care plans.
• Administer medications and treatments with accurate documentation.
• Perform clinical assessments and monitor changes in residents’ conditions.
• Assist physicians during rounds and exams.
• Communicate effectively with residents’ families and care team members.

About the Role:

As a Per Diem LPN, you will provide hands-on nursing care to residents while supporting the clinical and leadership needs of the unit. You will collaborate with nursing staff and interdisciplinary teams to ensure residents receive compassionate, high-quality care. Our Per Diem LPN will be responsible for:
