Rochester Regional Health is seeking a Licensed Practical Nurse (LPN) for the Edna Tina Wilson Living Center in Rochester, NY. This per diem role requires delivering professional patient care to long-term and post-acute residents and supervising CNAs.

You will ensure accurate documentation and follow physician orders, with flexible scheduling and opportunities to advance within the organization. The position requires NY LPN licensure, BLS certification, and prior LPN experience.

Qualifications

• Graduate from an accredited School of Practical Nursing.
• Current LPN licensure in State of New York.
• BLS Certification from the American Heart Association (AHA).

Responsibilities

• Timely, accurate medication administration and required documentation.
• Perform treatments and document progress per physician orders.
• Ensure concise resident care documentation in the electronic medical record.
• Initiate appropriate care pathway tools.
• Direct CNA team to complete quality personal care per care plans.
• Ensure CNA documentation is accurate and complete.
• Receive and relay reports to staff per protocols.
• Transcribe orders and follow up to institute orders timely.
