Job summary

Live Well Homecare Agency, LLC is seeking an Licensed Practical Nurse Navigator to guide patient populations through complex care pathways, focusing on access, referral accuracy, sequencing, and continuity across services and settings.

You will review referrals, collaborate with healthcare providers, communicate with patients about their care, coordinate activities and educational series, and help remove system barriers to timely follow-up.

Qualifications

• Graduate from Specialty Training Program in Nursing (Required).
• Minimum of 2 years LPN experience.
• Strong clinical knowledge, communication, and care coordination skills.
• Ability to collaborate across disciplines and serve as a patient advocate.
• Proficiency in developing protocols, managing referrals, and maintaining systems for follow-up.

Responsibilities

• Review and triage referrals for appropriateness and urgency, ensuring necessary records are obtained.
• Collaborate with healthcare providers to coordinate and implement patient care plans.
• Communicate with patients via various methods regarding their care.
• Coordinate patient activities, support groups, and educational series.
• Support internal referral retention and capture, promote patient navigation procedures, and develop screening tools.
• Address patient concerns related to access, scheduling, and communication.
• Conduct medical record reviews and track patient progress from referral through care completion.
• Assist patients in navigating system barriers and ensure timely follow-up and surveillance.
