Job summary

Live Well Homecare Agency, LLC is seeking a Licensed Practical Nurse (LPN) to provide patient-centered care under RN supervision, performing assessments, procedures, medication administration, and port access within scope.

You will collaborate with the healthcare team, support care plans, educate patients and caregivers using teach-back, participate in staff development, and ensure accurate documentation and regulatory compliance.

Qualifications

• LPN license and BLS certification required.
• Experience in oncology or outpatient settings preferred.
• Strong assessment, organizational, and problem-solving skills.
• Excellent communication and computer skills.
• Ability to delegate tasks appropriately and work collaboratively with team members.

Responsibilities

• Coordinate direct patient care and document in medical records.
• Collaborate with the healthcare team to develop and revise individualized care plans.
• Educate patients and caregivers using teach-back methods.
• Attend staff meetings, in-services, ongoing education, and serve as a preceptor for new LPNs.
• Ensure accurate documentation and adhere to regulatory guidelines.
