Job summary

Altru Health System in Grand Forks, ND is seeking a Licensed Practical Nurse (LPN) to perform and document assessments, support admission and discharge planning, and deliver patient care. You will coordinate with the multidisciplinary team to ensure safe, effective, and timely nursing interventions.

The role requires ND LPN licensure or a compact state license, completion of an LPN program with IV therapy, and the ability to educate patients and families.

Qualifications

• Licensed Practical Nurse (LPN) in good standing with ND Board of Nursing or compact state.
• Completed LPN program with IV therapy included or eligible to obtain within 6 months.
• Ability to perform and document assessments and participate in discharge planning.
• Excellent teamwork and communication with multidisciplinary staff.

Responsibilities

• Assist RN or licensed practitioner in identifying patient needs and care goals.
• Gather and communicate patient responses to nursing interventions.
• Collaborate to modify care plans with licensed practitioners.
• Document nursing care timely and accurately.
• Contribute to discharge planning and patient education.
• Participate in multidisciplinary team to ensure integrated care.
• Identify early signs of clinical changes and report them.
• Delegate tasks safely to qualified staff.
• Maintain adherence to department standards of care.
• Support health history and admission processes.
