Job summary

Altru Health System in Grand Forks, ND is seeking a Licensed Practical Nurse (LPN) to perform and document assessments appropriate to licensure, ensure health history and admission processes, provide patient care, and participate in discharge planning and education.

You will collaborate with the multidisciplinary team, adjust plans of care with licensed practitioners, communicate patient progress, and document nursing care accurately to support quality and continuity of care.

Qualifications

• Must be licensed as LPN in North Dakota or in a Compact State.
• Must obtain Minnesota licensure within 12 months of start date.
• Proficiency in English reading, writing and speaking.

Responsibilities

• Assist licensed practitioner in identifying patient needs and goals.
• Document responses to nursing interventions accurately.
• Collaborate with the multidisciplinary team to implement care plans.
• Carry out nursing interventions and prescribed regimens in a timely, safe manner.
• Support discharge planning and patient education.
• Ensure effective communication of patient progress to team members.
