Job summary

Montefiore Medical Group is seeking an experienced LPN to join a collaborative care team. You will provide hands-on nursing support under RN and physician supervision, administer medications, monitor vitals, and assist with treatments in a fast-paced clinical setting.

Responsibilities include accurate documentation, room turnover, and patient education. A NY LPN license, BLS, and infection control certification are required. This is a full-time, weekday schedule supporting patient-centered care.

Qualifications

• New York LPN license required.
• BLS certification required.
• Infection Control certification required.
• High School Diploma or GED required.

Responsibilities

• Provide hands-on care under RN/physician supervision, including medications administration and vitals monitoring.
• Accurately document patient information, vitals, and interventions with high accuracy.
• Prepare and sterilize exam rooms and manage supplies between sessions.
• Collaborate with the healthcare team to support discharge planning and patient education.
• Deliver patient and family teaching and discharge instructions.
