Job summary

WeCare at The Green Home Care Center, located in Wellsboro, PA, is seeking licensed practical nurses (LPNs) for a part-time 2PM-10:30PM shift to provide essential nursing care for residents.

Responsibilities include health monitoring, medication administration, assisting with daily activities, documentation, and collaboration with the nursing team to ensure quality resident care. WeCare promotes teamwork, a healthy work–life balance, and professional growth.

Qualifications

• Must hold a valid LPN license in the State of PA.

Responsibilities

• Conduct thorough assessments of each patient’s medical status upon admissions and throughout the course of treatment.
• Provide regular patient status updates.
• Assist in implementing patient’s treatment plan under direction of the Registered Nurse.
• Complete required documentation in an accurate and timely manner.
• Supervise, administer and or assist with medications.
• Provide direct care of residents as necessary.
• Follow the plan of care for each resident.
• Ensure the residents care standards are maintained.
• Report to the Director of Nursing.
