Job summary

TEEMA Solutions Group is seeking a Licensed Practical Nurse (LPN) in Missouri to provide compassionate, patient-centered care under supervision of an RN or physician. The role includes administering medications, monitoring conditions, and documenting care.

The ideal candidate will have an accredited LPN program graduate, active LPN license in Missouri, and be prepared to work in a team-based setting across LTC/SNF or clinical environments with adherence to infection control and HIPAA.

Qualifications

• Graduate of an accredited Practical Nursing program.
• Current LPN license in the state of practice.
• BLS certification current or per facility policy.
• Experience in LTC/SNF, rehabilitation, clinic, or acute care preferred.

Responsibilities

• Provide direct nursing care per individualized care plans.
• Administer medications and treatments as prescribed.
• Monitor, assess, and report changes in patient condition.
• Assist with admissions, transfers, discharges, and ongoing care.
• Maintain accurate and timely clinical documentation.
• Collaborate with physicians, nurses, and the care team.
• Educate patients and families within scope of practice.
• Follow infection prevention, patient safety, HIPAA, and standards.
• Perform other duties as assigned.

Requirements

• Current Licensed Practical Nurse (LPN) license in the state of practice.
• Current BLS certification preferred or required per facility policy.
• Recent experience in long-term care (LTC), skilled nursing (SNF), rehabilitation, clinic, or acute care preferred.
