Job summary

TEEMA Solutions Group in Milbank, SD is seeking a Licensed Practical Nurse (LPN) to provide compassionate, patient-centered care under supervision. You will administer medications, document care, and collaborate with the interdisciplinary team to promote safe, high-quality outcomes.

The ideal candidate has graduated from an accredited LPN program, holds an active state license, and may have BLS certification. Experience in LTC/SNF, rehab, or clinic settings is preferred.

Qualifications

• Graduate of an accredited Practical Nursing program.
• Current LPN license in the state of practice.
• Current BLS certification preferred or required per facility policy.
• Recent LTC/SNF/rehabilitation/clinic/acute care experience preferred.

Responsibilities

• Provide direct nursing care according to individualized care plans.
• Administer medications, treatments, and therapeutic interventions as prescribed.
• Monitor, assess, and report changes in patient condition to the Registered Nurse or provider.
• Assist with admissions, transfers, discharges, and daily patient care activities.
• Maintain accurate, timely, and complete clinical documentation.
• Collaborate with physicians, Registered Nurses, and the interdisciplinary healthcare team.
• Educate patients and families within the scope of practice.
• Follow infection prevention, patient safety, HIPAA, and regulatory standards.
• Perform other duties as assigned.

The LPN administers medications and treatments, monitors patient conditions, performs routine nursing procedures, documents care, and collaborates with the interdisciplinary healthcare team to promote safe, effective, and high-quality patient outcomes.

Requirements

• Graduate of an accredited Practical Nursing program.
• Current Licensed Practical Nurse (LPN) license in the state of practice.
• Current BLS certification preferred or required per facility policy.
• Recent experience in long‑term care (LTC), skilled nursing (SNF), rehabilitation, clinic, or acute care preferred.
