• Apply nursing interventions and meds under the direction of the RN.
• Provide direct patient care within the primary nursing model to ensure continuity of care.
• Perform nursing procedures and treatments according to policies and Nurse Practice Act.
• Assess and reassess patient pain and document changes.
• Communicate patient condition changes to RN promptly.
• Deliver care with pathophysiological and psychosocial rationale.
• Establish rapport with patient and family; introduce self as the nurse.
• Educate patient/family and provide discharge instructions as needed.
• Document nursing interventions and patient responses in medical records.
• Delegate responsibilities appropriately and participate in performance improvement.
