Job summary

Tufts Medicine seeks an LPN Outpatient to work under the RN, using the nursing process to plan, implement, and evaluate care. You’ll collaborate with the care team to provide health information within a supportive care framework, collecting data with the team to identify health goals and ensure patient and family care aligns with organizational values.

Responsibilities include administering medications, documenting outcomes, teaching patients, and supporting discharge instructions.

Qualifications

• Licensed Practical Nurse in MA with current license.
• BLS/CPR certification required; CPR instructor preferred.
• Experience dependent on unit requirements.

Responsibilities

• Apply nursing plan of care and administer medications under RN supervision.
• Provide direct patient care within the primary nursing model.
• Perform nursing procedures safely per policies and Nurse Practice Act.
• Assess and reassess pain; document management and changes.
• Communicate changes in patient condition to RN immediately.
• Base interventions on pathophysiology and psychosocial rationale.
• Build rapport with patient and family; provide teaching.
• Educate discharge instructions and ADL needs to patients/families.
• Document all interventions and patient responses in the chart.
• Delegate tasks appropriately according to staff capabilities.
• Participate in performance improvement activities.
• Maintain professional demeanor; commit to lifelong learning.
• Collaborate with peers to foster a positive work environment.
• Report observations to physicians or nurse in charge; accurate records.
