Job summary

Sunny Ridge Health and Rehabilitation Center is seeking a skilled Licensed Practical Nurse (LPN) for full-time and part-time PM/DAYS shifts. The candidate will administer medications, monitor residents, and provide comprehensive nursing care.

Ideal candidates will hold a Wisconsin LPN license and demonstrate the ability to direct Certified Nursing Assistants. We offer an excellent benefits package, including 401K and PTO, in a supportive work environment focused on patient care.

Qualifications

• Current Wisconsin License as a Licensed Practical Nurse.

Observes, reports and charts on physical and mental condition of residents. Administers medications and treatments as prescribed by the physician and permitted by licensing regulations.

Observes and reports changes in condition of residents to the Registered Nurse and attending physicians.

Receives and executes the written or verbal orders of physicians. Assists residents with activities of daily living as assigned. Monitors use of special equipment (i.e. oxygen, intravenous, etc). May perform other care duties/assignments per facility needs.

Current Wisconsin License as a Licensed Practical Nurse. Ability to monitor and direct Certified Nursing Assistants. Understands and effectively carries out oral and written instructions. Understands the concept of comprehensive nursing care.
