Responsibilities

• Accepts assignments of clients from the R.N. Case Manager.
• Provides nursing care through the utilization of home assessments, nursing diagnoses, and physician-approved care plans.
• Performs treatments ordered by the physician and delegated by the R.N. Case Manager.
• Observes signs and symptoms and reports them to the R.N. Case Manager.
• Notes changes in physical or emotional status of clients.
• Promotes client and caregiver independence in self-care.
• Utilizes electronic documentation of client care; complies with agency policies regarding timeliness in submission of documentation and synchronization of data.
• Follows agency policies regarding universal precautions and safety guidelines.
• Fulfills other assignments as requested by the R.N. Case Manager, VP of Clinical Services, or Director of Professional Services.
