LPN-- Saturday & Sunday 10p-6a

Homecare with Heart is offering you the most competitive pay scale in the area. We're looking for compassionate and experienced nurses to provide care for our clients in their homes. As a Home Care Nurse, you will be responsible for overseeing the quality, expert care of your assigned homecare patients. Starting rate for LPN: $26 per hour.

This position will be serving Trumbull County.

Responsibilities

What You’ll Do:

• Administer medication in accordance with standards of practice.
• Encourage social/rehabilitative skills and promote self-care and independence.
• Perform client treatments according to the Plan of Care.
• Assist clients in learning appropriate self-care techniques.
• Prepare clinical and progress notes to document care provided.
• Monitor, document, and report any changes in client condition to the Clinical Director/Clinical Supervisor.
• Promote activities to ensure a safe and healthy environment.
• Observe aseptic technique during client procedures/care.
• Collaborate with team members to develop and improve the Plan of Care.
• Assist clients in achieving goals outlined in the Plan of Care developed by the RN.
• Maintain productivity standards set by the agency.
• Keep licenses, CEUs, and employment requirements up to date.
• Participate in client care meetings, in-services, orientation, and staff meetings.
• Provide services aligned with the agency’s philosophy, standards, policies, and scope of practice.
• Participate in Performance Improvement initiatives and adhere to HIPAA guidelines.
• Perform other duties as assigned.

• Valid State LPN License.

• Ability to read and understand documents such as the Plan of Care, flow charts, client charts, policies, and procedures.
• Proficiency in using a computer, laptop, or tablet.
• Ability to think independently, recognize potential problems, gather information, and know when to seek supervision.
• Commitment to excellent customer service and providing exceptional client care within agency rules and program guidelines.
