Job summary

Interim HealthCare Inc. is seeking Licensed Practical Nurses (LPN) and Registered Nurses (RN) for in-home patient care. Evening and night shifts are available with 8- or 12-hour options, coordinated around nurse availability.

You will deliver compassionate, patient-centered services in clients' homes across the service area. Responsibilities include assessments, medication administration, wound care, and care coordination with the wider clinical team.

Qualifications

• Graduate of a practical nursing program (LPN/LVN).
• Current unrestricted LPN/LVN license in the state of assignment.
• Current CPR/AED/BLS/First Aid certification.
• Reliable transportation to care sites.

Responsibilities

• Provide in-home direct patient care per plan of care and nursing scope.
• Assess patient conditions, document vitals, and report changes.
• Administer medications, insulin, IV fluids, and document thoroughly.
• Inspect wounds, change dressings, assist with hygiene.
• Collaborate with providers and care team to ensure outcomes.

As a Licensed Practical Nurses (LPN) and Registered Nurse (RN) you will:

• Provide in-home direct patient care according to the provider's prescribed plan of care (and nursing scope of practice) while closely monitoring progress/recovery
• Assess and document patient conditions and report change as needed (vitals, blood/stool/urine, glucose, etc.)
• Administer medication, insulin, and IV/fluids, documenting thoroughly
• Inspect and care for wounds, changing dressings and assisting with personal hygiene
• Assess patient for further injuries, infections, bedsores, etc. and deliver appropriate treatments
• Collaborate with providers, aides, and allied health professionals to ensure exceptional patient outcomes

To qualify for a Licensed Practical Nurse (LPN) position with us, you will need:

• Educational: Graduate of a practical (vocational) nursing program for LPN/LVN and has a current license as a LPN/LVN in the state position is located
• Licensure: Current unrestricted license to practice as Licensed Practical Nurse (LPN) in the state associated with this position
• Current CPR/AED/BLS/First Aid certification
• Reliable transportation to/from care sites and/or work locations
• Practical trach and/or ventilator experience preferred, not required
