Qualifications

• Accurate charting of assessments, interventions, and patient status in EHR.
• Monitor patient status and promptly report to supervising clinician.
• Adhere to infection control, safety protocols, and quality standards.
• Demonstrate accountability for personal performance and meet performance metrics.
• Collaborate with interdisciplinary team to optimize patient outcomes.
• Maintain on-site inventory and proper storage of clinical supplies.
• Identify process improvement opportunities and contribute ideas.

Responsibilities

• Provide Licensed Practical Nurse care within scope of practice on-site.
• Assess patient needs and document findings in medical records.
