Job summary

LEXINGTON is seeking a Nurse Supervisor to provide direct nursing care and oversee day‑to‑day activities of nursing assistants in a long‑term care facility. This role ensures adherence to federal, state, and local standards and collaborates with the Director of Nursing to maintain high quality care.

Responsibilities include directing nursing staff, maintaining policies, guiding discharge planning, and coordinating services across shifts.

Qualifications

• Must possess a Nursing Degree or be a graduate of an approved LPN/LVN program.
• Must hold a current, active nursing license (RN/LPN/LVN) in the state.
• Current CPR certification and knowledge of PCC or equivalent care systems.

Responsibilities

• Direct the day‑to‑day nursing assistants’ functions in accordance with current rules and facility guidelines.
• Ensure nursing staff comply with facility policies and procedures.
• Review and revise policies, procedures, and job descriptions as needed.
• Plan shifts, services, programs, and activities with nursing staff and support personnel.
• Maintain Nursing Service Procedures Manual and ensure compliance with its contents.
• Prepare and file required nursing records, reports, and forms.
• Coordinate and supervise resident discharge planning and coordination of care.
• Provide leadership and supervisor support to nursing personnel on your unit/shift.
