The Citadel of Sterling is seeking a Licensed Practical Nurse (LPN) to join our team in Sterling, IL. The LPN will care for residents, perform assessments, monitor vitals, and participate in care plan implementation and evaluation.

Responsibilities include ensuring safety, infection control, and clear communication with residents, families, and staff. A nursing degree or diploma and an IL license are required; long-term care experience is preferred.

Qualifications

• Current LPN license in Illinois.
• Nursing degree or diploma from an accredited program.
• 1 year of experience preferred.
• Long-term care experience preferred.

Responsabilités

• Provides licensed care to residents as ordered by physician and per standards.
• Performs assessments/observations and updates care plans.
• Monitors vital signs and administers IV therapy within license.
• Documents care accurately and communicates with families and staff.
• Maintains a safe environment and infection control practices.
• Coordinates with other staff to meet resident needs.
• Ensures adherence to HIPAA and facility policies.
