Freedom Behavioral Hospital is seeking an experienced LPN to join our team in Louisiana. The LPN will provide nursing interventions, administer medications, document care, and support admissions and discharges. The role requires a current LPN license in the state and CPR/BLS certification.

Qualifications

• Graduate of an approved Practical Nursing or Vocational Nursing program.
• Current, unrestricted LPN license in the state of practice.
• Current CPR/BLS certification (provider or renewal if needed).
• One year of LPN experience preferred in skilled nursing, long-term care, rehab, or healthcare setting.

Responsibilities

• Provide nursing interventions per charge nurse on shift.
• Administer medications as prescribed and document timely in medical record.
• Assist with admissions and discharges; coordinate care with team.
• Transcribe and audit orders, ensure accuracy in medical records.
• Advise patients and families; educate as indicated.
• Communicate with nursing team to meet patient needs.
• Support activities of daily living and promote independence.
• Liaise with medical staff about patient issues; document findings.
• Ensure infection control and safety guidelines are followed.
• Participate with Service Development for timely admissions.
• Collaborate as part of multidisciplinary team to meet patient goals.
