Job summary

Pavilion Healthcare is seeking a Licensed Practical Nurse (LPN) to join our team in providing licensed nursing care to residents as ordered by physicians. The LPN will document and communicate care, supervise CNAs, and ensure safety and accuracy in medication administration.

This role requires attention to detail and adherence to regulatory standards. The LPN will assess residents, monitor vital signs, transcribe orders, and maintain comprehensive nursing notes.

Qualifications

• Current CPR certification required.
• Possess a current license to practice in the State as an LPN.
• 1 year of experience preferred.
• Long-Term care experience preferred.

Responsibilities

• Provide licensed care to residents as ordered by physician.
• Provide and receive nursing reports during shift reporting.
• Maintain clean and sanitary work areas and care environments.
• Supervise day-to-day certified nursing assistant services for the unit.
• Assess residents and review care plans for appropriate care.
• Take and record vital signs and administer medications as ordered.
• Transcribe physician orders to MAR and resident charts.
• Document nursing notes reflecting care provided and resident response.
• Complete forms on admission, transfer, and discharge.
• Address safety risks and maintain a safe environment.
• Submit shift reports and recommendations as required.
• Suggest care plan changes to the IDT per standards.
