Job summary

Abode Care Partners is seeking a Nurse Care Manager (LPN) to deliver patient-centered care in collaboration with field providers. You will assess patients, plan care, educate patients and families, and coordinate with the care team to implement clinical plans.

The role requires an active LPN license, strong communication, and proficiency with EHRs. This full-time position offers pay at USD 24–26 per hour and opportunities to work across multiple KY locations.

Qualifications

• Associate degree preferred
• Active LPN license (compact license preferred)
• Excellent communication skills
• Ability to work independently and as part of a clinical team
• Proficient with Microsoft Excel for tracking and reporting
• Experience with EHR software and Health Information Exchange

Responsibilities

• Conduct initial and ongoing clinical assessments and document findings in the EHR
• Develop and update individualized care plans with the care team
• Coordinate care with providers, family members and caregivers
• Educate patients and caregivers on treatment plans and medication management
• Handle medication-related inquiries and document all interactions
• Review clinical orders for accuracy and compliance with provider instructions
• Ensure HIPAA compliance and participate in QA checks
• Collaborate with Care Facilitators for scheduling and non-clinical inquiries
