Qualifications

• Current LPN license in the state of Nebraska.
• 0 years of experience in long-term care or a related field.
• Ability to work varying shifts, including nights, weekends, and holidays.
• Strong communication and interpersonal skills.
• Ability to lift, push, and pull up to 50 pounds, and stand for extended periods.

Responsibilities

• Provide direct patient care, including administering medications, performing vital signs, and assisting with daily living activities.
• Supervise and coordinate the activities of Licensed Practical Nurses (LPNs) and Certified Nursing Assistants (CNAs) during the night shift.
• Develop and implement individualized care plans for residents, in collaboration with the interdisciplinary team.
• Conduct rounds and monitor resident conditions, reporting any changes or concerns to the DON or MD.
• Maintain accurate and up-to-date records, including documentation of patient care and medication administration.
• Participate in quality improvement initiatives and maintain compliance with regulatory standards.
