Job summary

Banner Health’s Payson Multi-Specialty Clinic is seeking an LPN to provide nursing care under the direction of an RN or physician. You will observe, document, and contribute to the plan of care, using your nursing skills to support patient assessments and safe care delivery.

The schedule is Monday–Friday, 8am–5pm. You’ll work in a fast-paced setting within Banner Medical Group, collaborating with a multidisciplinary team to promote quality patient outcomes and continuity of care.

Qualifications

• Must hold a current LPN/LVN license in the practicing state.
• BLS certification is required.
• Completion of a state approved IV therapy program may be required.
• Banner Registry and Travel positions require a minimum of one year experience in an acute care hospital setting.
• Experience in an acute care setting within the past 12 months as an LPN/LVN in the specialty area.
• Arizona Fingerprint Clearance Card required for Banner Home Care AZ.

Responsibilities

• Observe, compile and report the patient’s physical, psychological, social, spiritual, educational, developmental, cultural and discharge planning needs.
• Contributes to plan of care under direction of registered nurse, including discharge planning.
• Implements care based on physician orders and nursing plan of care.
• Report changes in patient condition to RN/physician and document patient progress.
• Maintain accurate patient records and communicate with the care team.
