Wellpath seeks a Licensed Practical Nurse (LPN) to support patient care in our facilities. Under RN supervision, the LPN assesses patients, supports education and nursing activities, and helps implement treatment plans.

The role involves administering medications (including IV therapy if certified), performing diagnostic tests, participating in safety procedures, and documenting encounters using SOAP notes. We offer training, career growth, and a supportive team in a mission-driven environment.

Formación

• Graduate from an accredited school of nursing.
• Prefer a minimum of one year clinic experience.
• Have and maintain current licensure as a Licensed Practical/Licensed Vocational Nurse within the state of employment; CPR certification required.

Responsabilidades

• Under RN supervision, assess patients' physical, psychological, and social needs and contribute to individualized treatment planning.
• Administer medications, including IV therapy if certified, and perform diagnostic tests according to protocols.
• Collaborate in controlled substance counts and respond to codes/health emergencies within standard guidelines.
• Assist in medical procedures, implement nutrition and therapeutic plans, and document nursing encounters using SOAP charting.
• Attend mandatory staff meetings and training sessions to maintain high-quality patient care.
