Job summary

COMHAR is seeking a compassionate Licensed Practical Nurse (LPN) to provide person-centered care for individuals with intellectual and developmental disabilities in the Philadelphia area. The role includes medication administration, health monitoring, documentation, and coordination with the care team.

The position is full-time in the Philadelphia, PA 19124 area with a 8:30 AM–4:30 PM schedule on weekdays and rotating on-call coverage for weekends.

Qualifications

Active LPN License.
At least two (2) years of mental health experience preferred.
Valid driver’s license required.
Physical examination including TB test and Hep B.
Act 31 Mandated Reporter Training.

Responsibilities

Administer medications and supervise self-administration when allowed.
Immediately report medication errors to supervisor.
Review laboratory results and notify providers timely.
Infection control and adherence to hygiene protocols.
Coordinate with interdisciplinary treatment teams and document in medical records.
Provide direct services including personal care and daily living tasks.
Lead activities and participate in community integration as needed.
