Job summary

Harbor House of Wheeling IV LLC is seeking a Licensed Practical Nurse (LPN) to join their care team in Wheeling, Illinois. The LPN will provide licensed nursing care, administer medications, perform assessments, and document care plans under physician orders and facility standards.

The role requires strong communication, infection control, and the ability to multi-task in a busy memory-care setting. CPR certification and a current LPN license are required, with long-term care experience

Qualifications

• Current CPR Certification required.
• Possess a current license to practice as an LPN in the state.
• 1 year of experience preferred.
• Long-Term care experience preferred.

Responsibilities

• Provides licensed care to residents as ordered by physician and in accordance with standards.
• Provides and receives nursing reports at shift change.
• Maintains clean and sanitary work areas and treatment spaces.
• Directs and supervises CNA services for unit to ensure compliant care.
• Performs resident assessments and reviews care plans for adequacy.
• Takes vital signs and administers medications/treatments as ordered.
• Transcribes physician orders to charts and MARs.
• Documents nursing notes reflecting care provided and resident response.
• Completes admission, transfer, and discharge forms as required.
• Maintains safety and addresses hazards in the resident environment.
• Provides reports and recommendations on shift activities, as required.
• Ensures care plans are implemented and updated as needed.
