Vicar's Landing Oak Bridge Memory Care in Ponte Vedra Beach, FL seeks a compassionate Licensed Practical Nurse (LPN) to deliver direct, person-centered care in a Montessori-inspired memory care environment. You’ll work with residents, families, CNAs, and the interdisciplinary team to support clinical wellness, comfort, and independence.

Qualifications

• Active Florida LPN license and current CPR/BLS certification.
• Strong clinical assessment and observational abilities.
• Excellent verbal and written communication; thorough documentation.
• Ability to exercise sound nursing judgment per care plans.
• Experience supervising or supporting CNAs is beneficial.

Responsibilities

• Provide direct nursing care per physician orders and care plans.
• Administer medications and treatments with accurate documentation.
• Monitor residents for condition changes and notify RN/Director of Nursing.
• Perform routine procedures: wound care, catheter care, specimen collection, vital signs.
• Support Montessori-inspired memory care and resident engagement.
• Collaborate with CNAs, nurses, physicians, therapists; assist admissions transfers discharges.
• Maintain compliance: documentation, infection control, safety protocols.
