Job summary

Qualifications

Responsibilities

• Document patient care and education per policy.
• Administer medications within LPN scope and hospital policy.
• Coordinate discharge planning and patient care plans.
• Communicate with Clinical Coordinator and Charge RN as needed.
• Support infection control and quality improvement initiatives.
