Job summary

LiveWell Homecare Agency is seeking an LPN Medication Nurse to provide licensed nursing support in a senior living environment, ensuring safe medication administration and accurate clinical documentation. You will administer medications, monitor residents, document in MAR/TAR in real-time, and contribute to care plan updates while maintaining regulatory compliance and resident safety.

Applicants must hold an active LPN license with CPR and First Aid; prior senior care or medication

Qualifications

• Active LPN license in good standing with CPR and First Aid certification.
• Experience in senior care or medication administration is preferred.
• Strong assessment, observation, and documentation skills.

Responsibilities

• Administer medications and treatments following physician orders and the Six Rights of Medication Administration.
• Accurately document in MAR/TAR in real-time and support care plan updates.
• Monitor resident symptoms, report adverse reactions or refusals promptly, and observe physical, emotional, or cognitive changes.
• Maintain medication carts, conduct narcotic counts, and ensure compliance with regulatory standards.
• Conduct Service & Health Updates (SHUs) and transcribe physician orders.
