Job summary

LiveWell Homecare Agency is seeking an LPN Medication Nurse to provide licensed nursing support in a senior living setting. You will administer medications, perform health monitoring, and document care accurately to ensure resident safety.

In this role you will follow physician orders, maintain MAR/TAR records in real time, observe resident changes, and coordinate with the care team to prevent medication errors while upholding regulatory standards in Fresno, CA.

Qualifications

• CPR and First Aid certification.
• Strong assessment and documentation skills.
• Experience in senior care or medication administration preferred.

Responsibilities

• Administer medications and treatments following physician orders and the Six Rights of Medication Administration
• Accurately document in MAR/TAR in real-time
• Monitor symptoms, report adverse reactions or refusals promptly
• Conduct Service & Health Updates (SHUs) and transcribe physician orders
• Observe and report physical, emotional, or cognitive changes in residents
• Maintain medication carts, complete narcotic counts, and ensure compliance with regulations
