The Avenue Care & Rehabilitation Center in Cleveland, OH is seeking compassionate Floor Nurses to provide direct care to residents with routine medical needs. Licensed LPN or RN preferred; you will follow individualized care plans and collaborate with a care team.

Responsibilities include administering medications, monitoring residents, assisting with admissions, and ensuring safety and regulatory compliance. We foster a team environment and prioritize resident-centered care.

Qualifications

• Current LPN or RN license in the state of employment.
• CPR certification (AHA, Red Cross, or equivalent).
• One year of long-term care experience preferred.
• Strong clinical, communication, and organizational skills.
• Ability to lift up to 50 lbs and work flexible hours.

Responsabilités

• Deliver nursing care based on individualized care plans.
• Administer medications and treatments as prescribed.
• Monitor and document resident conditions and responses.
• Assist with admissions, discharges, and transfers.
• Supervise nursing assistants and collaborate with care teams.
• Participate in care planning and assessments.
