Responsibilities include documenting in EMR, administering medications, monitoring residents, and coordinating with CNAs and physicians. Per diem shifts with 6a-6p or 6p-6a available.

Qualifications

• 1 year of experience as an LPN in long-term care.
• Must have reliable transportation and consistent attendance.
• Able to lift at least 50 pounds and perform related physical tasks.
• Legally authorized to work in the United States.
• Willing to complete background and drug screening.

Responsibilities

• Use EMR to record patient information and vital signs.
• Administer medications and treatments per orders and policies.
• Monitor resident health and report changes promptly.
• Provide direct nursing care within scope of practice.
• Assist with assessments and care plan implementation.
• Document medications, treatments, and resident responses; collaborate with care team.

Responsibilities

An LPN picking up shifts through Elite Medical Staffing at a long-term care facility may be expected to:

• Utilize EMR to record patients’ medical information and vital signs
• Administer medications and treatments in accordance with physician orders and facility policies
• Monitor resident health status and promptly report significant changes in condition
• Provide direct nursing care and clinical support within scope of practice
• Assist with resident assessments and implementation of individualized care plans
• Accurately document medications, treatments, observations, and resident responsesCollaborate with CNAs, medication aides, physicians, and other healthcare team members
• Communicate effectively with residents, families, and healthcare providers regarding resident needs
• Maintain compliance with facility policies, infection control standards, and regulatory requirements
• Communicate concerns or issues to onsite supervisor and agency
• Other duties as assigned

Qualifications

• Minimum 1 year of experience working under an active LPN license
• Minimum 1 year of experience working in long-term care
• Must have reliable transportation to assigned facilities
• Must maintain consistent attendance and punctuality
• Able to push, pull, and lift at least 50 pounds
• Able to be standing and walking with frequent bending, reaching, and lifting with proper body mechanics
• Must be legally authorized to work in the United States
• Willing to complete background and drug screening in accordance with applicable laws and regulations

Credentials Required

• Active and unencumbered XX LPN License or Compact LPN License
• Government-Issued Photo ID or Passport
• CPR/BLS
• Tuberculosis (TB) Test
• Flu Vaccine (during Flu season)
• Covid-19 Vaccine (religious/medical exemptions accepted)
• Resume
• Additional credentials may be required based on facility placement
