Job summary

The Licensed Practical Nurse (LPN) will work in a Long-Term Care/Special Needs Facility. The role involves providing care for residents with various diagnoses, including long-term and dementia care for adults 18+.

You will participate in care planning and collaborate with interdisciplinary teams to deliver comprehensive care. Responsibilities include med and treatment administration, wound care, blood glucose monitoring, IV meds, insulin, and routine nursing duties, with on-site pharmacy

Qualifications

• Minimum 1 year of nursing experience.
• Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) required.
• BLS Certification required.
• Must-have: PCC experience.
• Experience measuring wounds.

Responsibilities

• Provide med administration and treatments including wound care.
• Monitor blood glucose and insulin administration as prescribed.
• Handle IV medications, TPN, and tube feedings as ordered.
• Record and chart care plan details and vitals.
• Collaborate with RNs, therapists, social workers and other staff.
