Qualifications

• Active and unencumbered LPN license (FL or Compact)
• Hospice experience preferred
• Reliable transportation to assigned facilities
• Consistent attendance and punctuality required
• Ability to push/pull and lift 50 pounds
• Prolonged standing/walking with bending/reaching
• Authorized to work in the United States
• Willingness to complete background and drug screening

Responsibilities

• Utilize EMR to record patient information and vital signs
• Administer medications and treatments per orders and policies
• Monitor resident conditions and report changes to the clinical team
• Record health information and vital signs accurately
• Provide direct nursing care including wound care within scope
• Assist with assessments and care plan implementation
• Document medications, treatments, observations and responses
• Collaborate with CNAs and healthcare team to ensure quality care
• Maintain facility policy, infection control and regulatory compliance
• Communicate concerns to supervisor and agency
• Other duties as assigned

Responsibilities

An LPN picking up shifts through Elite Medical Staffing at a hospice facility may be expected to:

• Utilize EMRto record patients’ medical information and vital signs
• Administer medications and treatments in accordance with physician orders and facility policies
• Monitor resident conditions and promptly report significant changes to the appropriate clinical team
• Measure, record, and maintain accurate resident health information and vital signs
• Provide direct nursing care, including wound care and other treatments within scope of practice
• Assist with resident assessments and implementation of individualized care plans
• Accurately document medications, treatments, observations, and resident responses
• Support and collaborate with CNAs and other healthcare team members to provide quality care
• Maintain compliance with facility policies, infection control standards, and regulatory requirements
• Communicate concerns or issues to onsite supervisor and agency
• Other duties as assigned

Qualifications

• Minimum 1 year of experience working under an active LPN license
• Minimum 1 year of experience working in hospice
• Must have reliable transportation to assigned facilities
• Must maintain consistent attendance and punctuality
• Able to push, pull, and lift at least 50 pounds
• Able to be standing and walking with frequent bending, reaching, and lifting with proper body mechanics
• Must be legally authorized to work in the United States
• Willing to complete background and drug screening in accordance with applicable laws and regulations

Credentials Required

• Active and unencumbered FLLPN License or Compact LPN License
• Government-Issued Photo ID or Passport
• CPR/BLS
• Tuberculosis (TB) Test
• Physical

• Additional credentials may be required based on facility placement
