Job summary

Covenant Living and CovenantCare at Home are seeking licensed practical nurses to deliver high-quality care in assisted living, skilled nursing and memory care facilities. You will monitor staff performance, coordinate with physicians, and provide direct resident care per standards and regulations.

Responsibilities include supervising nursing assistants, training new staff, communicating changes to the care team, and ensuring accurate medication and procedure adherence.

Qualifications

• Graduate of an approved school of licensed practical/vocational nursing.
• Current license, in good standing, as an LPN/LVN in the State.
• One year nursing experience.

Responsibilities

• Assist routine daily direction and monitor the work of the nursing assistants on the unit, assuring a high standard of nursing care and satisfactory completion of tasks.
• Assist in the orientation of new LPN/LVNs and nursing assistants and reinforce training standards to maintain a high quality of care.
• Provide explanation of nursing procedures and assistance to staff to assure ongoing understanding and training.
• Communicate on a regular basis with the Nurse Manager or Director of Nursing regarding observations, and suggestions of problem areas, resident and employee activities, concerns, needs, conditions, etc.
• Observe and evaluate residents' health, mental and emotional conditions and communicate significant changes to appropriate staff.
• When indicated is responsible for notifying the physician and family members according to prescribed procedures.
• Provide direct patient care including catheter placement, colostomy care, suction, dressings, and NG tubes.
• Participate in shift handovers to ensure proper resident information transfer.
• Assist in admission, orientation, and observation of new residents, ensure physician orders for diets, therapy, medications are transcribed and communicated.
