Shift Details:
- Four 10 hour shifts. 06:00AM-16:30PM.
- Every other weekend and holidays as needed.
Licensed Practical Nurse (LPN) Duties & Responsibilities:
- Administers medications, vaccines, and injections safely according to provider orders and medication rights.
- Provides direct patient care based on individualized plans of care.
- Performs medication refills and point-of-care testing, including phlebotomy and specimen collection.
- Provides routine treatments such as wound care, enemas, colostomy care, topical medications, and surgical preparation.
- Monitors patient conditions and reports observations and treatment outcomes to RNs and Advanced Practice Providers.
- Responds to patient emergencies and provides appropriate interventions in accordance with BLS standards and scope of practice.
- Accurately documents all patient care, treatments, and encounters in the electronic health record.
Minimum Qualifications:
- Current, valid, and unrestricted WI license/certification in good standing, as required for the position.
- Minimum of one (1) yearof recent work experience as an LPN within the last 3 years
- A degree from an accredited school of Nursing
- Active BLS
- Copy of current, completedPrimary Source Verification (PSV) from the appropriate licensing board or verification agency, if required.
- Copy of all current required certifications and compliance documents (e.g., .BLS, N95 fit test, immunization records, TB test, or other client-specific requirements).
